Using Apps to support Long Term Conditions

Manage Your Health App

The Manage Your Health app uses computer-generated characters, interactive quizzes and information resources to help patients with long-term conditions such as Asthma, Chronic Obstructive Pulmonary Disease (COPD) and Type 2 Diabetes. Other conditions covered by the app are Atrial Fibrillation, Chronic Kidney Disease, and Hypertension.

The app can be used on Android or Apple devices, and includes answers to commonly asked questions as well as using computer-generated characters to demonstrate key points to help manage your condition, such as effective inhaler technique.
